Medicare 101
As a federal health insurance program, Medicare is notable for its longevity and stability over the past five decades. It consistently meets the essential healthcare needs of senior citizens and individuals with certain disabilities or illnesses.
Original Medicare is administered by the federal government and typically involves a fee for each service. Below are the basic guidelines for its operation:
Factors That Affect Original Medicare Out-Of-Pocket Costs
- Whether you have Part A and/or Part B. Most people have both.
- Whether your doctor, other healthcare provider, or supplier accepts assignment.
- The type of healthcare you need and how often you need it.
- Whether you choose to get services or supplies Medicare doesn't cover. If you do, you pay all the costs unless you have other insurance that covers it.
- Whether you have other health insurance that works with Medicare.
- Whether you have Medicaid or get state help paying your Medicare costs.
- Whether you have a Medicare Supplement Insurance (Medigap) policy.
- Whether you and your doctor or other healthcare provider sign a private contract.
Medicare Supplement Insurance (Medigap)
Medicare Advantage Plans
Since the 1970s, Medicare beneficiaries have had the option to receive their Medicare benefits through private health plans. Medicare Advantage plans, also called Medicare Part C, are health insurance plans approved by Medicare and run by private insurance companies. A Medicare Advantage plan is required to provide all Medicare Part A and Medicare Part B benefits and may include prescription drug coverage.
Most Medicare Advantage Plans also add additional benefits like dental or vision coverage, health club memberships, medical transportation, or coverage for hearing aids. Although some require the beneficiary to pay a premium, they are considerably lower than most Medicare supplements.
Medicare Part D Prescription Drug Plans
When to Enroll in Medicare Part D
If you are registered for Medicare Part A and/or Part B and reside within your plan’s service area, you are eligible to enroll in the Medicare drug benefit (Part D) during your Initial Enrollment Period (IEP). Your IEP for Part D typically aligns with your Part B enrollment period. This spans seven months, including three months before, the month of, and three months after your Medicare eligibility month. For instance, if your 65th birthday is on May 15, making you eligible for Medicare, your IEP would run from February 1 to August 31.
Note: If you are disabled and approaching 65, you will qualify for a new Part D Initial Enrollment Period (IEP). This IEP extends for seven months, covering three months before you turn 65, the month of your 65th birthday, and the three months following. If you previously incurred a premium penalty for late enrollment in Part D, this penalty will cease once your new Part D IEP starts.
If you enroll in a Medicare drug plan within the three months prior to becoming eligible for Medicare, your coverage will commence the month you become eligible. If you enroll during your birthday month or within the following three months, your drug coverage will start on the first day of the month after you sign up. We advise you to enroll early in your IEP to ensure your coverage starts as soon as you’re eligible.
If you miss enrolling in a Medicare drug plan during your Initial Enrollment Period, you may have to wait until the Fall Open Enrollment to join. This period starts on October 15 and ends on December 7. Any changes or enrollments made during this time take effect on January 1. Additionally, you might incur a premium penalty for late enrollment.
You may also have a Special Enrollment Period to enroll in Part D under exceptional circumstances, including if:
- you get extra help, or
- you lose employer drug coverage.
